Note what appears to be a hole in the floor at the bootom left of the door.

 

Compare the man standing on the ground with the size of the hole in the airframe. 

 

 

I wonder what was lost through the hole at 30,000 feet?

 

   

Quite a bit of junk fell out from the ceiling.
